2) Fill in all your basic information.

3) Select your donation type as “Monthly”.

4) Choose your own donation amount.

5) In the section of donation method, please select “Credit Card”, then fill in all the related information.

6) Please submit the completed (a)Donation Form to BEAM by mail, e-mail or fax.

3) Besides, please fill in all the related information and sign the (b)Autopay Form.

4) Please submit the completed (a)Donation From and (b)Autopay Form to BEAM by mail, e-mail or fax.
